Another case of cellular explosion [updated]

Back in August
we mentioned that a women in Austria had her Nokia catch fire. The company blamed her 3rd-party replacement battery. There has been another case of cell phone fire, this time in Omaha:
bq. "When the phone exploded, the battery blew out of the compartment and ended up lodged in the car seat..."
The phone was made by Kyocera
, and it is not clear if a replacement battery was on board or not. However, since the phone was only 2 days old it's unlikely that anything not from the factory was onboard.
Kyocera has stated that it is stopping shipments of the phone (KE400/KX400 series) worldwide until it can clarify what went wrong.
